Syracuse Community Health Center (SCHC) is a patient-centered medical home which means our focus is on you and your health. In a patient-centered medical home, a team of health professionals works together to create an environment where you have access to quality health care that provides you with the best possible health outcomes. Your primary care provider leads the care team which may include nurses, medical assistants, receptionists, specialists and other health care professionals working with you as the center of the team. Within our facility, we offer a variety of other medical services that complement your primary care to make it easier for you to receive comprehensive care.
