Currently seeking full-time nurse practitioner to join St. Luke's Home Health to enhance a population health program. This position is focused on reducing re-hospitalizations and emergency room utilization. The work is performed in close collaboration with other providers, system service lines, and population health initiatives. Examples include but not limited to; Remote Patient Monitoring, St. Lukes Health Partner case management, cardiopulmonary rehab, and assisted living facilities. We are looking for a practitioner who is experienced in Internal Medicine, Family Medicine and/or Home Care. The Advanced Practice Provider is responsible for providing high-quality healthcare directly in the patients home, through the diagnosis and treatment of chronic conditions such
