Practice Description

Fairview Health Services is seeking a Family Medicine Physician to join our primary care practice at M Health Fairview Clinic - Riverside. M Heath Fairview Clinic - Riverside is a well-established clinic with a diverse and loyal patient population. Centrally located in the Cedar-Riverside neighborhood on the University of Minnesota Medical Center West Bank Campus, the clinic draws from both well established urban communities as well as new immigrant communities. Covered parking and food trucks over the lunch hour are some of the perks of being on campus.

Practice Details:

• Currently with 10 providers, we are recruiting to replace a provider who is transitioning to an addiction medicine practice.
• Patient encounters average 15-20 patients per day.
• 36 patient contact hours per week.
• Call Frequency averages 1-2 nights per month and taken from home. Call is phone only and answered by a nurse line. No inpatient responsibilities.
• Openings: 1.0 FTE
• Work with a complete team including MTM Pharmacy and Behavioral Health on site as well as a history of collaboration with Addiction Medicine and Pain Medicine located on the same floor as the clinic.
• Care Coordination services are available, including social work, nursing and a community health worker.
• Nurse triage on site during all clinic hours.
• Fully integrated Electronic Medical Record (EPIC).

Qualifications

• Graduate of an accredited ACGME/AOA Family Medicine residency training program.
• BC/BE with the ABMS or AOA.
• Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges if applicable.
