Nemours is seeking a Developmental Med APRN to join our Wilmington Team!
This position is responsible for Developmental Medicine practice and provides direct and indirect care to patients and their families in the Developmental Medicine Division. APN will collaborate with other members of the healthcare team and obtain a comprehensive developmental, health and medical history. APM will order and interpret age-appropriate and condition-specific screening tests, laboratory tests, and diagnostic procedures. APN will formulate differential diagnosis, and participate in formulation of a holistic plan of care. They will coordinate care and make appropriate referrals, as well as maintain appropriate and necessary documentation in the medical record.
Essential Functions:
- Practices collaboratively with physicians and other health care providers to provide high quality health care to children.
- Performs appropriate assessment of patient/family upon referral/admission.
- Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care.
- May use prescriptive authority if appropriately credentialed to do so. Collaborates inter-professionally as needed. Identifies patient/family need for further resources and initiates appropriate referrals.
- Assists the Staff Nurse in formulating the Nursing Care Plan and setting priorities based on the needs of patient and family.
- Coordinates Nursing Plan for Discharge in collaboration with the Staff Nurse and other members of the health care team.
- Provides patient/family teaching based on identification of learning needs.
- Evaluates compliance with the prescribed treatment regimen through telephone calls, family/patient assessments and outpatient visits as needed.
Requirements:
- Master of Science in Nursing
- Three years of related clinical experience
- Active Delaware (or compact state) Registered Nurse Licensure required
- Active Delaware Advanced Practice Nurse Licensure with Prescriptive Authority
- Delaware Controlled Substance Registration (CSR)
- Federal DEA registration with Delaware Address
- Current Pediatric primary care certification
- Current AHA Healthcare Provider BLS Certification required
- PA licensed with prescriptive authority to be obtained upon hire
