Summary
VA NWIHCS is an integrated system that serves more than 148 -000 Veterans in 101 counties in Nebraska - western Iowa - Kansas - and Missouri. It includes a tertiary medical center in Omaha - 2 larger clinics in Lincoln and Grand Island (which includes a Community Living Center) - 5 community-based outpatient clinics - and one contract clinic.
Duties
- The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers
- This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest
- As an employee operating in a Whole Health System of care - you will operate in a model with three core elements - seeking to create a personalized health plan for each Veteran
- This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community
- This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being
- The Nebraska Western Iowa VA Health Care System- Omaha - NE is currently recruiting for a full time - permanent - Primary Care Physician Assistant
- Additional duties may be assigned by the Primary Supervisor Physician - appropriate Chief of Service or Chief of Staff - which may included provisions for special training to assure proficiency
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Preforms clinical procedures
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assess and adjusts the plan of care as needed to achieve optimal patient outcomes
- Work Schedule: Clinic hours Monday- Friday - 8:00am-4:00pm
- Additional information may be provided during interview process
- Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory - qualification - and performance requirements are met
- Selection at a lower grade level does not guarantee promotion to the full performance level.
