SummaryThis position will serve as an academic primary care provider (PCP) in a Patient Aligned Care Team (PACT) with a teamlet comprised of a provider (MD/DO) - RN - LVN - and a Clerk. The team will provide primary and acute care services in an outpatient setting to include chronic disease management and preventive health care. The academic PACT will also include resident physicians.QualificationsTo qualify for this position - you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement.
