Summary

This is OPEN CONTINUOUS ANNOUNCEMENT and will remain open until August 14 - 2026. The initial cut-off date for referrals of eligible application will be June 14 - 2026 with subsequent cut-off dates every month. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.

Duties

• Provide comprehensive primary care and preventative care to an established panel of primary care patients - under the Patient Aligned Care Team (PACT) model
• The type of care is served to the Veteran population with diseases consistent to those seen in the general population include but are not limited to the following: acute and chronic illness - women's health - gender specific - mental health - and preventive care - along with coordination of care delivery
• A major focus is on preventative medicine
• Function as a leader in the PACT approach in the delivery of care - which emphasizes a team-based approach to healthcare
• Participate in weekly team meetings and provide innovative management ideas for patient case reviews
• Duties include - but are not limited to - history taking - physical examination - ordering appropriate labs - diagnostic tests - and consultations needed
• Formulate multi-disciplinary treatment plans
• Perform medication reconciliation
• Document all medical information in an electronic medical record
• Other duties as assigned
