Summary
The Physician Assistant for Nephrology at the Durham VA Medical Center involves management of Veterans with hypertension and chronic kidney disease (CKD) in outpatient nephrology clinics at Durham VA Healthcare System. This PA will be fully qualified and certified to function in both a clinical and administrative role and in accordance with the guidelines and policies of the Durham Veterans Affairs Medical Center.
Duties
- The primary focus of practice for the Physician Assistant (PA) is the clinical management of healthcare for patients followed in the nephrology clinics.Major duties include but are not limited to: Provides comprehensive clinical management for Veterans with hypertension and chronic kidney disease (CKD) in outpatient nephrology clinics at Durham - Raleigh - Garner - and other existing or future Durham VA-affiliated nephrology clinics - including Telehealth nephrology clinics
- Reviews and triages new nephrology consults - manages outpatient follow-up (including post-discharge patients) - and identifies - diagnoses - treats - and refers patients in collaboration with the attending nephrologist and interdisciplinary team
- Manages complications and associated conditions of advanced CKD
- assists with dialysis access planning - dialysis modality selection - transplant referral - and arranges kidney biopsy and dialysis access procedures with Vascular Radiology and Surgery
- Provides clinical support to outpatient dialysis clinics - the inpatient nephrology consult service - the kidney transplant clinic - and the Durham VA Community Based Dialysis Program
- Ensures safe - high-quality care through accurate documentation - adherence to approved protocols - participation in quality improvement activities - and coordination with VA and community providers
- Serves as liaison for the Nephrology Section - assisting with clinic operations - community-based dialysis coordination - scheduling support - and completion of required clinical forms
- Provides patient - family - and staff education related to CKD - dialysis - and renal replacement therapy
- participates in nephrology clinical research as appropriate
- and supports staff development through in-service education while maintaining current knowledge of trends in dialysis and nephrology
- Participates in the multidisciplinary team to identify and screen patients for the Dialysis Program - monitor dialysis outcomes - facilitate patients' transitions across health care systems - and utilizes and promotes approved standards of care - procedures - methods - safety precautions - and protocols in the care of patients with CKD and those receiving dialysis in all settings
